- Inadequate lubrication: Use sufficient anaesthetic gel (at least 5-10ml); wait 2-3 minutes for it to work
- Wrong angle: The male urethra has two curves; hold penis at 90° (perpendicular) to straighten it
- Premature balloon inflation: Advance catheter fully until urine flows freely, then another 2-3cm to ensure balloon sits in bladder
- Sterility breaks: Once gloves are on, don't touch non-sterile surfaces; keep field tidy
- Forgetting foreskin: Always replace foreskin to prevent paraphimosis (painful, requires urgent reduction)
- Forcing catheter: If resistance is felt, do NOT force; withdraw gently and reassess (may need urology referral)
Complications to Discuss
Immediate: Pain, bleeding, urethral trauma, false passage
Early (hours-days): Catheter blockage, UTI, bladder spasms, catheter migration
